To provide the highest quality of care in environment for disabled adults that prioritizes support and accountability; ensuring that our clients are cared for, yet still allowed to explore and strengthen their independence so that they maintain their dignity in an atmosphere of encouragement, safety, and trust.
Pauline Transition House, Inc. (PTH) was founded by Teresa Hamm in February 2013 and is a nonprofit 501(c)(3) tax-exempt. located in Loganville Ga. This public charity was named after the late Pauline Hamm for her huge heart and giving spirit. PTH was originally established to offer refuge for homeless men and women in need of a safe environment to realign their lives by learning self-discipline, re-building their damaged self-esteem, and finding the courage to work on becoming self-sufficient once again. Today, PTH serves as a community-based.

PTH has spent its own funds making up the substantial difference in cost per month in order to continue operating and providing care to all clients. PTH needs your help to acquire adequate funding to meet this goal and continue our mission for many years to come.
